Introduction

 Morphological analysis is one of the oldest fields in cancer research; thus, this field possesses a wide range of knowledge that has been developed over decades. The division of molecular pathology aims to integrate this knowledge into the rapidly developing field of molecular biology to study the molecular biological characteristics of tumors from the morphological perspective.

The Team and What We Do

 Diagnosis is now made on the basis of various protein expressions and genetic abnormalities that go beyond morphology, and drugs that are highly effective against specific genetic mutations have been developed for some tumors. In the field of molecular pathology, we collaborate closely with the Department of Diagnostic Pathology at Central Hospital, and conduct research on the use of genetic alterations in diagnosis and the identification of genetic abnormalities that are useful in diagnosis, based on the questions raised in the process of diagnosis. We are also investigating the molecular mechanisms of tumorigenesis and progression based on the results of various biological research projects and cancer genome medicine.

Research activities

 Thoracic oncology group: EGFR mutations in lung adenocarcinoma are driver mutations recognized from a precancerous state, but we reported that driver mutations such as ALK, ROS1, and BRAF are also found in early-stage lesions with no difference in frequency. We also reported that NUT carcinoma lacks some characteristics as a squamous cell carcinoma.

 Gastrointestinal group: Our genetic analysis of intestinal adenocarcinoma with anastomosing glands of the stomach showed the frequent presence of RHOA mutations, which had been thought to be specific to diffuse-type gastric cancers. We also identified novel RSPO fusions in colorectal serrated lesions.

 Hepatobiliary pancreatic group: We showed that the prognosis was poorer in the high expression group of HLA class I antigens in pancreatic cancer cells than in the low expression group, suggesting that this might be due to the induction of immune checkpoint molecular expression by IFNγ resulting in an immune-permissive microenvironment. We reported a case where yolk sac tumors of pancreatic origin responded to chemotherapy for germ cell tumors.

 Head and neck cancer group: we reported that OLMF4, a molecule regulated by methylation, might be a new molecular marker for early-stage cancers based on comprehensive mRNA expression analysis and genomic methylation analysis of head and neck squamous cell carcinoma. Also, we identified novel fusion genes YAP1-MAML2 and YAP1-NUTM1 fusions in benign poroma and porocarcinoma, which are adnexal tumors of the skin.

Education

 We accepted two graduated fellows and trained them for cancer research.

Future prospects

 With advances in basic research and the spread of cancer genome medicine, fundamental genetic disorders have been discussed in clinical settings. Our research aim is to use our morphological approach as a means of assigning biological meaning/significance to individual diseases and obtaining background molecular features across the organs.
