Introduction
Remarkable progress has been made in not only endoscopic diagnosis but also endoscopic treatment. Our efforts will be focused on the development of new endoscopic diagnosis (e.g. molecular imaging, functional imaging) and treatment (e.g. full thickness resection), as well as effective training systems with industry-academia collaboration (Figure 1).
The Team and What We Do
Since fiscal year 2017, in the Division of Science and Technology for Endoscopy of the Exploratory Oncology Research & Clinical Trial Center (EPOC) at the National Cancer Center (NCC) Tsukiji campus, four teams were organized (diagnosis team, treatment team, training team, AI team) to develop innovative endoscopic diagnosis, treatment and training systems. We searched clinical needs, and then set up development themes for each team.

Research activities
1. In collaboration with Olympus Corporation, we have started the development of new endoscopic diagnostic, therapeutic and training equipment at Olympus Lab in the National Cancer Center Research Institute (NCCRI). In fiscal year 2019, we continued to develop innovative endoscopic equipment. Although the details are currently confidential, each team is involved in three or four research and development themes.
2. In collaboration with the Artificial Intelligence Research Center (AIRC), the National Institute of Advanced Industrial Science and Technology (AIST) (Professor Jun Sese), the NCCRI (Professor Ryuji Hamamoto), and the Division of Science and Technology for Endoscopy of the EPOC (Kashiwa campus: Professor Hiroaki Ikematsu), we have started the research and development of an endoscopic diagnosis support system on tumor invasion of gastrointestinal cancer using AI. 3. The patent application was filed jointly with Olympus Corporation for two patents: “Medical Stapler,”“Medical Stapler and Suturing Method,”“Fastening Device, Endoscopic System, and Suturing Method,”“Suture Method,”“Grasping Forceps and Suturing Method,”“Regarding the Field of View of Grasping Part,”“Regarding Advancement and Retraction Mechanism for the Ulcer Closure Device,” developed in the joint research with Olympus Corporation, and “Total Laminectomy Method,” jointly filed with Olympus Corporation. (Olympus Corporation has applied for six single patents.) In academic and research society presentations, we received the Best Presentation Award at the 2nd meeting of EFTR & Closure Conference (December 2020), and gave presentations at the 3rd meeting of the same society and the 112th meeting of the Kanto Branch of the Japanese Society of Gastrointestinal Endoscopy. Chronic animal experiments are currently underway (August 2022) on a prototype model with final specifications for closure of the defect hole after full-layer resection.
Education
Residents of the Endoscopy Division of National Cancer Center Hospital participate in the meetings of each development team and various development experiments to train and educate the next generation of people who will be responsible for endoscopic device development.
Future prospects
In our division, we would like to work on the development of innovative endoscopic diagnosis, treatment, and training systems in collaboration with industry, academia, and government in order to solve many current issues and future demands in daily clinical practice, and to disseminate them from Japan to the world.
