Introduction
Research is aimed at constructing evidence connected to the development of cancer prevention by clarifying the causes of cancer in humans using large-scale cohort studies.
Research activities
1) Traditional molecular epidemiologic studies to investigate the causes of cancer
We have conducted several traditional molecular epidemiologic studies to investigate the causes of cancer. By using pre-diagnostic plasma samples obtained from the Japan Public Health Center based prospective study (JPHC study), we conducted a case-cohort study to investigate the association between chronic low-grade inflammation, as reflected by C-reactive protein (CRP), and cancer risk. The multivariable-adjusted hazard ratio (95% confidence interval) for the top quartile of CRP was 1.28 (1.11-1.48) (P trend < 0.001) for overall cancer compared to the bottom quartile of CRP. Among site-specific cancers, higher CRP levels were associated with an increased risk of colorectal, lung, breast, biliary tract, and kidney cancer, and leukemia. Our consistent findings suggested that chronic low-grade inflammation influences the development of many cancers. We also conducted international collaborative genetic studies, and reported the constriction of polygenic risk scores for prediction of breast cancer risk in Asian populations, and the existence of a structural variant in the 2q35 breast cancer risk locus, which influences the activity of a long-range enhancer element.
2) Consortium on Large-scale Molecular Epidemiologic Studies in Japan
We leveraged J-CGE (Japanese Consortium of Genetic Epidemiology) studies to assess the associations of vitamin D concentration, lipid, and glycemic traits with colorectal cancer using a Mendelian randomization approach. A positive association of total cholesterol with colorectal cancer risk was indicated, whereas associations between other factors and colorectal cancer risk were not observed. We have now submitted these results to journals and conducted Mendelian randomization studies for other site-specific cancers. Furthermore, we created the J-CGE homepage and made a summary of statistics of meta-GWAS available to the public.
3) Innovative molecular epidemiologic studies to investigate the causes of cancer
We conducted a case-control study among a general population residing in Izu Oshima, and examined the association between Escherichia coli (E coli) containing polyketide synthase (pks) in the gut microbiota and colorectal neoplasia. We published the finding that the pks status of E coli was not significantly associated with the prevalence of colorectal adenoma lesions. Also, we evaluated somatic mutations in the KRAS gene among 655 colorectal cancer cases who were diagnosed in the Akita and Okinawa areas of the JPHC study. Then, we investigated whether the adverse effects of established risk factors, namely smoking tobacco and drinking alcohol, differed according to the existence of somatic mutations in the KRAS gene. Finally, we are conducting a new study to evaluate the emerging risk factors of heated tobacco products.
Education
We supervised the research of six graduate students and three of them have obtained doctoral degrees.
Future Prospects
By analyzing data from the epidemiological studies both within and outside Japan, we hope to contribute to developing evidence for personalized cancer prevention and promoting a lifetime of health.
