Introduction
The Division of Cohort Research aims to (i) create evidence for prevention of non-communicable diseases such as cancer, and (ii) conduct epidemiological research to extend the healthy life expectancy among people living in Japan using a population-based cohorts, domestic and international epidemiological research network framework, and the collaborative epidemiological research network project of the six national centers in Japan (6NCs) for advanced and specialized medicine - National Center Cohort Collaborative for Advancing Population Health (NC-CCAPH).
Research Activities
1. Using NC-CCAPH as a six national centers (6NC) research network to conduct collaborative epidemiological research on advanced and specialized medical care
NC-CCAPH is the core research project of the Division of Cohort Research. Initiated in 2017, the project aims to establish 6NC Epidemiological Research Collaboration Platform to conduct pooled analyses and mutual data analyses. As a part of this project, the collaborative analysis platform expanded on a secure cloud workspace and the analysis server was stabilized for smooth collaboration.
We have established a standardized evaluation criteria for the methodology. Based on this criteria, we have reevaluated the published recommendations on the association between several risk/preventive factors and diseases to extend healthy life expectancy in Japan. We will continue to update the evidence evaluation in 2023. The recommendations modified for the use of general public is being drafted.
2. Promotion of collaborative research among the six National Centers using ongoing cohort studies in the NCC
There are four Japan Public Health Center-based Prospective (JPHC) Study and seven JPHC Study for the Next Generation (JPHC-NEXT) collaborative studies that are ongoing or finished. For the JPHC Study, we reported that higher intake of total fruits as well as flavonoid-rich fruits were associated with decreased risk of depressive disorders. We also reported that among men who lived alone and women, having variety in the diet may prevent disabling dementia.
There are also five ongoing collaborative studies from cohort studies in the NC-CCAPH consortium. We have published the prevalence of family history of cancer in Japan as a pooled project.
3. JPHC Study / JPHC-NEXT
We have been conducting follow-up surveys and data analysis for the JPHC Study with 140,000 residents from all over Japan continuously since 1990. Using the JPHC Study, we reported that endoscopic screening may reduce the risk of advanced gastric cancer incidence and mortality. In another study, we found that alcohol consumption was associated with a pancreatic cancer risk among men who had no change in their alcohol-consumption habit over a five-year span. We also reported that heavy smoking (≥40 pack-years) was associated with an increased risk of renal cell carcinoma.
The baseline survey for the JPHC-NEXT was completed at the end of 2016, and we started the 5-year follow-up survey in 2016 (Table 1). Using JPHC-NEXT data, we reported that discretionary foods (such as miso soup) contribute the most to sodium (Na) consumption, but non-discretionary intake (such as pickled vegetables) may influence urinary Na excretion. The results suggested that reduction in the use of seasonings at home could be a recommendation to reduce Na consumption.
4. The conditions and factors of coronavirus infection among National Center staff as a part of Multicenter Observational Study
As part of a research project supported by the Japan Health Research Promotion Bureau, we are conducting a multicenter observational study of new coronavirus infections (COVID-19) among National Cancer Center staff at Tsukiji Campus. Web questionnaire surveys and blood antibody surveys were administered between June and July 2022 to over 500 participants. We are preparing to conduct a follow-up survey in 2023. There are two ongoing studies using data from the NCC and four studies using harmonized data of national centers.
Future Prospects
We hope to continue to expand and strengthen research collaboration networks to extend healthy life expectancy among Japanese, promote comprehensive inter-disciplinary disease prevention strategies, and provide scientific evidence to do so.
