Introduction

 Since 1962, our department has accumulated a vast array of clinical knowledge regarding musculoskeletal tumors in collaboration with radiologists and pathologists specializing in sarcomas, enabling us to offer well-organized treatment strategies to patients with various types of bone and soft tissue tumors. We have also conducted basic and clinical studies using accumulated clinical samples and data to establish novel diagnostic methods and therapeutic approaches for treating musculoskeletal tumors. In addition, we have emphasized clinical trials in three distinct but inseparable areas for bone and soft tissue tumors: surgery, chemotherapy, and radiation therapy.

The Team and What We Do

 Our department consists of six staff doctors and four residents. Outpatient consultations are held every weekday. A constant number of over 25 patients are hospitalized for surgery, chemotherapy, or radiation therapy. Six to 10 major surgeries for musculoskeletal tumors are routinely performed every week. The number of surgeries in our department in FY2024 was 348, of which 205 were for primary malignant bone and soft tissue tumors (Table 1).

 In collaboration with the Department of Plastic and Reconstructive Surgery, we conduct definitive tumor resection and reconstructive surgery for cases with extensive soft tissue defects or concomitant resection of critical neurovascular structures. In addition, we actively collaborate with other departments to perform complex surgeries that may be challenging to perform in other hospitals, such as combined sacral resection for radical resection of locally advanced rectal cancer (Department of Colorectal Surgery), retroperitoneal sarcoma surgery (Departments of Urology, Colorectal Surgery, Hepatobiliary and Pancreatic Surgery, and Gastric Surgery), and chest wall sarcomas (Departments of Thoracic Surgery and Breast Surgery).

 Regarding chemotherapy, we provide perioperative adjuvant chemotherapy for osteosarcoma and soft-tissue sarcoma, and chemotherapy for advanced stage bone and soft-tissue malignancies in collaboration with the Departments of Medical Oncology and Pediatric Oncology.

 We hold various cross-disciplinary conferences, including a sarcoma conference, a pediatric oncology joint conference, a medical oncology joint conference, a retroperitoneal sarcoma conference, a rehabilitation conference, and a bone metastasis conference.

Research Activities

 In basic research, in collaboration with the Division of Rare Cancer Research, National Cancer Center Research Institute, we have generated many sarcoma tissue mouse xenografts and new sarcoma cell lines, and have also performed comprehensive analysis of sarcoma-specific gene mutations and gene expression. In addition, we conducted several single-institution studies and domestic multi-institutional collaborative studies as part of our clinical research activities.

 The number of original papers published in English in FY2024 was 23.

 The research funds received in FY2023 were one MHLW Grants-in-Aid for Scientific Research, one Cancer Research and Development Fund, and one private research funds.

Clinical Trials

 Our institution has the largest number of malignant bone and soft tissue tumor cases in Japan and serves as a central facility for numerous clinical studies. In FY2024, we participated in 10 national multi-institutional collaborative studies, including two ongoing JCOG trials (one of which is an international JCOG–EORTC collaborative study for which we serve as the study office). In addition, we participated in six international collaborative studies, such as retrospective research on rare sarcomas and a registry study of metastatic bone tumors in Asia.

Education

 We actively accept young physicians from universities nationwide regardless of academic affiliation, and train human resources who will lead the treatment of bone and soft tissue tumors in Japan. In FY2024, four residents, including oncology residents and short-term residents, were accepted and trained.

 Young doctors who have completed the training program are guided to become bone and soft tissue oncology specialists (surgeon scientists) with excellent research minds. They are also guided in clinical research from planning to implementation, conference presentations, and paper writing.

 We encourage trainees to obtain various certifications during their training in the department, including certified oncologists. In FY2024, two trainees were certified as oncologists, three as bone and soft tissue oncologists by the Japanese Orthopaedic Association.

Future Prospects

 As Japan’s largest high-volume center for bone and soft tissue tumors, our institution fulfills its mission as the nation’s leading facility for the diagnosis and treatment of these diseases. Aiming to further improve treatment outcomes for malignant bone and soft tissue tumors, we are pursuing innovative approaches, including surgeries utilizing advanced technologies and the development of novel therapeutic agents. We also strive to take leadership in bone and soft tissue tumor research across Asia while actively promoting international collaborative studies with Europe and the United States. Furthermore, by serving as an educational and training institution where young physicians from across Japan gather to become specialists in bone and soft tissue tumors, we contribute to raising the overall standard of care for these diseases nationwide.
