Introduction
The Division of Cohort Research aims to (i) create evidence for the prevention of non-communicable diseases such as cancer, and (ii) conduct epidemiological research to extend the healthy life expectancy among people living in Japan using population-based cohorts, domestic and international epidemiological research network framework, and the collaborative epidemiological research network project of the six national centers in Japan (6NCs) for advanced and specialized medicine-National Center Cohort Collaborative for Advancing Population Health (NC-CCAPH).
Research Activities
1. Using NC-CCAPH as a six National Centers (6NC) research network to conduct collaborative epidemiological research on advanced and specialized medical care
NC-CCAPH is the core research project of the Division of Cohort Research. Initiated in 2017, the project aims to establish a 6NC Epidemiological Research Collaboration Platform to conduct pooled and mutual data analyses. As part of this project, we worked to strengthen the maintenance and stability of the collaborative analysis platform, including planning the replacement of remote terminals and the analysis server, as well as enhancing security measures to facilitate efficient collaboration.
In 2024, based on standardized evaluation criteria, we completed most of the evidence evaluation. This evaluation focused on the 19 diseases and 46 risk or preventive factors highlighted in the published recommendations for extending healthy life expectancy in Japan. The evaluation results will be published in 2025. The recommendations, adapted for public use, have been released. To facilitate broad dissemination, we considered establishing collaborations with municipalities and academic societies, as well as further developing related materials.
2. Promotion of collaborative research among the six National Centers using ongoing cohort studies in the NCC
There are four Japan Public Health Centre-based Prospective (JPHC) Studies and seven JPHC Study for the Next Generation (JPHC-NEXT) collaborative studies. Using the JPHC Study, we reported that individuals who continued to live alone had a higher risk of suicide compared to those who lived with others.
Using the JPHC-NEXT, we reported that higher intake of sugar-sweetened beverages, fruit and vegetable juices, and sugar-sweetened coffee was associated with an increased risk of depression, in contrast to higher intake of black coffee, which was associated with a lower risk.
Within the NC-CCAPH consortium, eight projects are in progress. In one pooled analysis, we reported that multimorbidity in midlife was associated with a higher risk of all-cause and cause-specific mortality.
3. JPHC Study / JPHC-NEXT
We have been conducting follow-up surveys and data analysis for the JPHC Study with 140,000 residents from all over Japan since 1990. This year, we reported that changes in the health status of close relatives affected individuals' mortality; specifically, having a spouse diagnosed with cancer was associated with an increased risk of death from external causes, particularly with suicide among men. We also examined the relatively rare cancer of the head and neck region and found a U-shaped association between BMI and cancer risk in the Japanese population, with higher risks at both low and high BMI levels, especially evident among non-smokers.
The baseline survey for the JPHC-NEXT was completed at the end of 2016, and we initiated the 5-year follow-up survey in 2016 and then the 10-year follow-up survey in 2021 (Table 1). Using the JPHC-NEXT data, we evaluated the validity of self-reported cancer screening and health checkup histories by comparing questionnaire responses with official records. Moderate agreement was observed overall; however, discrepancies were frequent for lung cancer screening, which was highlighted as an important consideration for analyses. Similarly, high concordance was observed between self-reported or interview-based histories of Helicobacter pylori eradication and claims data.

In addition, large-scale molecular epidemiology cohorts, based on baseline data, launched case-cohort studies for gastric, hepatobiliary-pancreatic, and breast cancers. Using whole-genome, exome, and targeted sequencing, we further advanced the risk evaluation of genetic variants related to breast and gastrointestinal cancers.
4. The conditions and factors of coronavirus infection among National Center staff as a part of Multicenter Observational Study
We are participating in a multicenter observational study of COVID-19 supported by the Japan Health Research Promotion Bureau. As a part of the study, we are conducting an observational study of new coronavirus infections (COVID-19) among National Cancer Center staff at the Tsukiji Campus. Web questionnaire surveys and collection of blood samples for antibody measurement were conducted between July and August 2024, including approximately 310 participants. We are preparing to conduct a follow-up survey in 2025. Currently, 18 research projects are ongoing using harmonized data from all six National Centers.
Education
We supervised the research of domestic and international Master's and PhD students. We accept postgraduate students from the Graduate School of Medicine, Yokohama City University, and the Graduate School of Humanities and Sciences, Nara Women's University.
Future Prospects
We will continue to develop cohort infrastructures, strengthen collaboration with domestic and international institutions, and generate evidence to extend healthy life expectancy through cross-disease prevention. We will also update our operational systems to further promote the use of existing resources.
