新型コロナウイルス感染症等の病院内伝播を防ぐためご協力をお願いします

新型コロナウイルス感染症等の病院内伝播を防ぐために以下のご協力をお願いいたします。

1. 患者さんおよびご家族は、マスクのご着用をお願いします。
2. 以下の（1）および（2）に該当する方は総合受付にお申し出ください。
   （1）発熱（37.5℃以上）、呼吸器症状（咳・呼吸困難等）がある
   （2）2週間以内に以下のいずれかに該当する場合
• 新型コロナウイルス感染症患者、またはその疑いがある患者との接触

また、必要時は体温測定や問診をさせていただくことがありますのでご協力をお願いいたします。

In due respect to global efforts to contain the spread of new coronavirus, the National Cancer Center of Japan requests your kind cooperation for the welfare of our patients.

1. We ask patients and accompanying family members/friends to please wear masks at our hospitals.
2. If any of the following are observed, please inform our staff at the information counter:

(1)Fever (over 37.5 degrees celsius), respiratory symptoms (coughs, breathing difficulties)

(2)Any within the past two weeks:

• Contact with patients infected or suspected with COVID-19.
   

When required, we may ask to take body temperature or ask relative questions. Your kind understanding and cooperation is very much appreciated.
